Assumption of Risk & Release of Liability

I understand that participation in dance classes, rehearsals, performances, camps, workshops, private lessons, and other activities offered by M.A.D Dance involves physical activity and carries inherent risks, including but not limited to slips, falls, muscle strains, sprains, fractures, or other injuries.

By choosing to participate, I voluntarily accept these risks.

I confirm that I (or my child) am physically able to participate in dance activities. I understand that it is my responsibility to consult a physician if I have any medical concerns and to notify M.A.D Dance of any condition, injury, allergy, or limitation that may affect participation.

M.A.D Dance is committed to providing a safe, supportive, and professional learning environment. However, accidents may occur despite reasonable care and supervision.

To the fullest extent permitted by applicable law, I voluntarily release and hold harmless M.A.D Dance, its owners, instructors, employees, volunteers, contractors, and affiliates from liability for personal injury, illness, property loss, or damages arising from participation in any M.A.D Dance activity, except where caused by gross negligence or willful misconduct.

I understand that this Release of Liability applies to all M.A.D Dance programs, including but not limited to:

• Group Classes

• Private Lessons

• Camps

• Workshops

• Performances

• Competitions

• Special Events

• Off-site Activities

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Agreement.

Parent / Guardian Consent (Participants Under 18)

If the participant is under 18 years of age, I certify that I am the parent or legal guardian of the participant.

I have read and understood this Agreement, consent to my child's participation in M.A.D Dance activities, and agree to the terms of this Release of Liability on behalf of both myself and my child.

Class Cancellation Policy

Group Classes

• Group classes may be cancelled up to 6 hours before the scheduled class time.

• Late cancellations or no-shows will result in one session being deducted from your package.

• Online booking closes 7 hours before class begins.

• If M.A.D Dance cancels a class after bookings have closed, any deducted session will automatically be credited back to your account.

Private Lessons

Private lessons require at least 24 hours' notice for cancellation or rescheduling.

Late cancellations or no-shows will result in the scheduled lesson being deducted from your package.

Group Programs

Group programs follow a fixed schedule and cannot be cancelled individually.

Any planned break or schedule adjustment must be approved by all participants and requested at least 24 hours in advance.

Refund Policy

All purchases are final.

Tuition, memberships, class packages, camps, workshops, private lessons, merchandise, and other M.A.D Dance products or services are non-refundable and non-transferable.

Refunds, credits, or extensions will not be provided for:

• Missed classes

• No-shows

• Late cancellations

• Scheduling conflicts

• Personal circumstances

• Dissatisfaction with the experience after participation

Please review all class schedules, cancellation policies, and program details before making your purchase.

Photography & Media Release

Unless otherwise requested in writing, I grant M.A.D Dance permission to photograph, film, or record myself (or my child) during classes, performances, competitions, workshops, or events.

I understand these materials may be used for educational, promotional, or marketing purposes, including M.A.D Dance's website, social media platforms, printed materials, and other publications, without additional compensation.
